Thank you for registering for our upcoming Medication Aide Class at Triad Medical Academy.   Please read the instructions carefully to complete the registration process.  If you have submitted your documents already, thank you.  If not here are further instructions.

Transcripts

We will need a copy of your high school OR college OR GED transcripts for your file.  It does not have to be official. You should get them mailed or emailed to you (preferably emailed) and then bring a copy to our office.  Here are a few options to obtain your transcript:

  1.     You can call or visit your high school or college directly to order your transcripts.
  2.     If you went to high school in Guildford County you may order one at this website : https://guilfordnc.scriborder.com
  3.     For a High School transcript, you may order one from this site:  https://www.needmytranscript.com
  4.     If you are currently enrolled in, taken some college courses, or graduated from college you may order your transcripts at National Student Clearinghouse (if your school participates) Click here:   https://tsorder.studentclearinghouse.org/school/select

Other Documents

We also need a copy of your social security card and drivers license or valid ID.

Submitting your documents

You may upload your documents on our website at www.triadmedicalacademy.com click the Other tab and on the drop down click, submitting your documents. Or you may bring them into our office by calling 336-510-2582 or email us at info@triadmedicalacademy.com to make an appointment.

Dress Code and what to bring.

You may wear any color nursing uniform or scrubs to class and closed toe shoes.  Also bring a pen, highlighter, and a notebook or notebook paper to class.

Your GPS may take you past our office due to the large office park.  If you are traveling west on Wendover, Turn Left on Clifton Rd, Make your first right on Oak Branch Drive, (at Logan Systems Building) Then Make your 1st Right in the Parking Lot, and our building is the first building on the left. You will see the #2 on the building.

No Make-up Days

There are no make up days for this course. You must attend all days to pass class or re-register and pay tuition again.
