Please read the instructions below carefully, to complete the registration process.
Transcripts (*ALL Transcripts must state a diploma or degree was awarded. Incomplete transcripts will not be accepted.)
We will need a copy of your high school, college OR GED transcripts for your file. It does not have to be official. You should get them mailed or emailed to you, and then bring a copy to our office. Or, request they be emailed to us directly, at info@triadmedicalacademy.com. Here are a few options to obtain your transcripts:
- You can order your transcripts directly from your high school or college.
- If you went to high school in Guildford County you may order one at this website: https://guilfordnc.scriborder.com
- For a High School transcript, you may order one from this site: https://www.needmytranscript.com

TB Test
A negative TB Test or acceptable X-Ray or Screening, completed within the last year, is required. If needed, we also offer a TB Test for $20. Click the link below, if you need a TB Test.

Background Check
All Nurse Aide I students are required to pass a criminal background check for Clinical rotation. You will sign a background check authorization form, at your Admission Document appointment.
COVID Vaccine Record
We also need a COVID vaccine record or an Exemption Form.
If you choose not to get a COVID vaccine you must complete an Exemption Form, in our office. You will be required to wear a mask to Clinical if you are exempted.
Also, if you test positive for COVID, before attending Clinical, you cannot participate in Clinical, on that rotation.
Uniform
It is required that you wear solid navy blue nursing scrubs to Clinical, with no designs of any kind. You may obtain one at any Walmart, Target, or Expressions Uniforms in Greensboro, at 2254 Golden Gate Drive. Or, you may visit any other nursing uniform shop locations. You may wear any solid color scrubs to class.

From I-40, turn Right on to Wendover Avenue, then Right on to Clifton Rd. Take the first Right on to Oak Branch Drive. Then take the first Right in to the Parking Lot. Our building is the first building on the Left. You will see the number 2 on the building. We are in Suite C.
Make-up Days for CNA Course
You are allowed to miss up to 11 hours of Lecture/Lab. Any make-up days will be announced in class.
