Transcripts

We will need a copy of your high school, GED or college transcripts for your file.  It does not need to be an official copy.  

ALL transcripts must state a diploma or degree was awarded. Incomplete transcripts will not be accepted. 

You should request the transcript to be mailed or emailed to you, and then bring a copy to our office. Or, request they be emailed to us directly, at info@triadmedicalacademy.com  Here are a few options to obtain your transcripts:

1. You can call or visit your high school or college directly to order your transcripts.
2. If you went to high school in Guilford County you may order one at this website : https://guilfordnc.scriborder.com

TB Skin Test

A negative TB Skin Test or an acceptable X-Ray or Screening, completed within the last year, is required.  You may ask your employer for a copy of your TB Skin Test or Screening, if it was required for your job. If needed, we also offer a TB Skin Test for $20.

Criminal Background Check

All Nurse Aide students are required to pass a criminal background check for clinical rotation. You will sign a background check authorization form, at your Admission Document appointment.     

 

COVID Vaccine Record

We also need your COVID vaccine record or an Exemption. If you have a medical or personal exemption you will complete a form in our office. You will be required to wear a mask to clinical if you are exempted.  Also, you will be tested for COVID before attending clinical, and if you are positive you cannot participate in clinical on that rotation.       

Uniform

Triad Medical Academy requires that you wear a solid navy blue nursing uniform/scrubs to clinical. No designs of any kind are allowed. You may purchase them at any Walmart, Target, or Expressions Scrubs & Shoes, located at 2254 Golden Gate Drive in Greensboro, or from any other nursing uniform shop. You may wear any solid color uniform/scrubs to class.

If you are traveling from I-40 on Wendover Ave, turn Right on to Clifton Rd. (First traffic signal passed Carmax.) Take the first Right on to Oak Branch Drive, (At the Logan Systems Building) Then take the first Right in to the Parking Lot. We are in the first building on the Left. Building 2, Suite C.
